Onboarding Mentorship Agreement
Purpose: This Mentorship Agreement is meant to ensure mentees and mentors develop a mutual
understanding of expectations from the beginning of their relationship. Additionally, it documents the
agreed upon goals and parameters that will serve as the foundation for the mentoring relationship.
Although the thoughtful completion of this form is a requirement, it is understood that items will change
and adjust naturally to fit the needs of both parties as the mentoring relationship grows.
This agreement will help each mentee/mentor pair:
● Establish communication and meeting expectations
● Identify goals for this mentoring relationship
● Outline skill areas to be enhanced or developed through this partnership
● Understand confidentiality of the other mentorship
As a mentee, I agree to do the following:
1. Meet regularly with my mentor and maintain frequent communication.
2. Look for multiple opportunities and experiences to enhance my learning.
3. Review my progress as I work towards my identified goals.
4. Maintain confidentiality of our relationship.
As a mentor, I agree to do the following:
1. Provide guidance, oversight, and encouragement.
2. Provide feedback regarding the mentorship contract, progress, and experience.
3. Meet in person or communicate regularly with my mentee to review their progress and help them
work toward identified goals.
4. Maintain confidentiality of our relationship.
Confidentiality: Both the mentor and mentee acknowledge and agree to maintain the strictest
confidentiality with respect to any information shared during the mentoring relationship. Mentor and
mentee will not share private information about each other or any other participant unless given
participant’s written consent to share. There are a few exceptions to this policy, as outlined below:
● Confidentiality obligations shall not apply to information that is or becomes generally available to
the public other than because of a disclosure by participant or was known by participant prior to
joining the mentorship.
● If either party becomes aware that a participant breaches the code of conduct in a serious
manner, harms themself or another person, or has been involved in any type of illegal behavior,
participant must report such information to the Mentorship Program Coordinator immediately.
If at any time during the duration of the mentoring contract one member of the mentoring pair does not
feel like the other is able or willing to fulfill the items in this agreement, please contact the Mentorship

Mentorship Termination
In the event that either party finds the mentoring relationship unproductive and requests that it be
terminated, that individual’s decision will be honored without question or blame. Please contact the
Mentorship Program Coordinator if you wish to terminate the mentorship.
